To begin a new claim, enter your Notice ID.
Your Notice ID is a unique, nine-digit number that appears at the top of each page
of the settlement notice that the Claims Administrator mailed you. Click here to
see exactly where the Notice ID appears on a sample notice.
Some Class Members, such as those who operate multiple offices, may receive more
than one settlement notice, and each notice will include a different Notice ID.
You only need to provide one Notice ID, and it does not matter which of your Notice
IDs you use. If you have more than one Notice ID, just pick one to enter below.
